Section 125 Plan

Through American Fidelity Assurance Company, PAA provides installation
of Section 125 plans. Under Section 125, the employees' portion of insurance
premiums can be purchased with pre-tax dollars, lowering the employees' federal withholding, state, local and FICA taxes. The employer also saves matching FICA taxes. The most important aspects of setting up payroll under Section 125 are proper documentation, regular reviews and updates. American Fidelity is PAA's recommended administrator for Section 125, providing all necessary documents and services at no additional cost to association members.
Disability
Disability programs protect an employee's income, while relieving the employer of any obligation to continue paying wages during a period of disability. The employer's cost is pre-determined, and the insurance company assumes the responsibility for claims determination and financial risk.
American Fidelity Assurance Company underwrites both short term and long term disability programs for PAA members. These plans are available as either an employer paid benefit or as a voluntary, employee-pay-all option.
Short Term Disability
Short term disability benefits begin with the first day a covered employee is unable to work due to a non-occupational accident and the eighth consecutive day if due to a non-occupational related sickness, to a maximum of six months.
Long Term Disability
Long term disability coverage allows an employee to safeguard his income if a disability extends over a long period of time.
Dental Insurance
Basic Program
A calendar year deductible of $25 per person ($50 annual per family) applies before benefits become payable at 100% of the maximum allowable charge (MAC). Basic and preventative dental services, such as exams, x-rays, cleanings, fillings, simple extractions and root canals are covered up to a maximum of $1,500 per person per calendar year. United Concordia participating dentists in Pennsylvania will accept payment for eligible services as payment in full (minus the deductible)
Full Program
In addition to basic benefits, this option also covers oral surgery and periodontal services at 100% MAC. Prosthetics, crowns, inlay and onlay restorations are also covered at 80% MAC. The calendar year deductible applies as with the basic plan. The full dental plan also includes a benefit for orthodontics for children only. Benefits are paid at 50% up to a lifetime maximum of $1,000 per child.
Vision Coverage
Davis Vision is a leading national administrator of routine vision care. Benefits include coverage for eye examinations, eyeglasses and contacts. Optional items such as designer frames, lens coatings and invisible bifocals are also available at low, discounted fixed fees.
Choose a participating network provider to get the greatest value and maximize your benefit dollars. The plan allows you to use out-of-network provides; however, a claim form would then be required and Davis Vision discounts would not apply.
PAA is pleased to offer two Davis Vision options.
Fashion Plan – Adults may receive an exam and new eyewear every 24 months; children under age 19 may receive an exam and lenses every 12 months, frames every 24 months.
Designer Plan – Adults and children may receive an exam and eyewear (including lenses and frames) or contacts in lieu of glasses every 12 months.
Health Insurance
Employees value health care coverage perhaps more than any other benefit, but employers are struggling with the escalating costs of providing quality programs.
PAA offers a number of insurance carriers in all areas of Pennsylvania with a wide range of benefit choices. These choices will enable employers to design benefit packages that meet their needs and fit their budget.
Health care plan options differ regionally across the state, and not all program types listed may be available in your geographic area, but all areas offer a variety of carriers and benefit options.
PPO (Preferred Provider Organization Programs)
These plans are designed for groups who want the greatest flexibility within a network program. Subscribers are free to see any health care provider within the network and receive the highest level of benefits or choose to see providers outside the network for a lower level of coverage. There is no need to select a primary care physician or get referrals for specialty care.
POS (Point of Service Programs)
Subscribers choose a primary care physician from an extensive network and get referrals for specialty care for the highest level of benefits. Members also have the option to use out of network providers at a reduced level of benefits.
HMO (Health Maintenance Organizations)
These plans generally offer subscribers lower out-of-pocket expenses with a network of healthcare providers specifically credentialed by the carrier to ensure quality care. Members choose a primary care physician who provides or coordinates the individual's health care needs. Most services must be performed or authorized by the primary care physician in order to be considered a covered service.
HDHP (High Deductible Health Plan) with HSA (Health Savings Account)
HSAs coupled with a qualified HDHP are becoming increasingly popular. HDHPs have higher deductibles and more out of pocket costs than other health care plans but can help the members take control of their health care expenses by offering catastrophic medical coverage at a lower premium. Once enrolled, account contributions can be made by an employee and/or employer to a tax-advantaged health savings account (HSA). The HSA can be used to pay for qualified health expenses tax free; the member owns the HSA and chooses how and when to use the funds.
HRAs (Health Reimbursment Accounts)
HRAs are employer established benefit plans which combine the protection of a deductible-based health plan with a health fund that pays for eligible services. Employers have control over HRA plan designs; employees cannot contribute to the HRA. Employers may choose higher deductible health plans at lower premiums and fund some of the deductible through an HRA account. This design can save premium dollars while still protecting employees from catastrophic out of pocket expenses.
